Review of Mental Health Clinical Pharmacists in Veterans Health Administration Facilities

The VA Office of Inspector General (OIG) conducted a healthcare inspection to assess Veterans Health Administration (VHA) facilities utilization of clinical pharmacists who work under a scope of practice in a mental health outpatient care setting.1 On August 1, 2018, the OIG published a report that identified risks to patients if mental health care provided by clinical pharmacists is not properly monitored.2 In that report, the OIG made recommendations pertaining to clinical oversight of clinical pharmacists and formal collaborating agreements between clinical pharmacists and independent providers at one medical facility. The seriousness of the risks identified in the prior report led the OIG to initiate a broader review of clinical pharmacists practice in mental health outpatient care settings.
