Each patient at Affinity Health System (AHS) – Ministry Health Care (MHC) or when appropriate, the patient’s representative (as allowed under Wisconsin law) has the right to: Be informedof theirpatient rightsuponadmissionto thehospital or inadvance of furnishingordiscontinuingcare, wheneverpossible. 2. Haveafamily member or representative ofhis/her choiceandhis/her ownphysicianinformed whenbeingadmitted to thehospital.
