From diabetes to COPD, even heart disease, a new program is helping patients better manage their chronic disease. “Chronic disease management works for anyone that has a chronic disease that they have trouble managing, or maybe they don't have trouble managing but they just want some more information, or they want some people to talk to about it,” said Lyn Carlson, a community care coordinator with Lee Health.
