Aurora Health Care and UnitedHealthcare to Improve Patients’ Care in Wisconsin with New Accountable Care Program

Aurora Health Care and UnitedHealthcare have launched an accountable care program designed to help improve people’s health and satisfaction with their health care experience. The new program will benefit the more than 40,000 people in the Milwaukee area enrolled in UnitedHealthcare’s Medicare Advantage plans. UnitedHealthcare and Aurora Health Care will work together to better coordinate patients’ care, using shared technology, timely data and information about health care visits to find more effective ways to treat patients. The accountable care program will also provide services to help patients manage their chronic health conditions by enhancing relationships with their primary care physicians and encouraging healthy lifestyles.
