Caregility and Hoy Health Collaborate to Support International Health Equity by Providing Digital Health Tools to Underserved Populations

Caregility, a company dedicated to supporting the delivery of care to patients wherever they are through the use of Caregility Virtual Care Platform, has announced a collaboration with Hoy Health, LLC, a first-of-its-kind digital health platform that provides comprehensive and integrated bilingual healthcare services to medically underserved patients. The partnership aims to overcome health equity gaps by increasing access to virtual care and remote patient monitoring resources explicitly designed for culturally diverse patient populations.

 

The collaboration combines Hoy Health's personalized Remote Patient Monitoring (RPM) Chronic Condition Management programs with Caregility's trusted virtual engagement tools to enable and ensure reliable patient access to primary care at any time and from any location. Peer health coaches are selected to support RPM programs based on their alignment with the patient's condition, demographics, culture, and language preferences. RPM and care management are provided in a virtual environment where patients can access education and support 24 hours a day, seven days a week.

 

Customers of Caregility have access to Hoy Health's bilingual RPM and chronic condition management program services. Customers of Hoy Health have access to telehealth services via the Caregility Virtual Care Platform, which helps in remote patient engagement. The companies will initially focus on providing digital health support to Spanish-speaking communities in the United States and Latin America.
