Health Alliance Improves Outcomes and Reduces Costs Using CareSignal’s Deviceless Remote Patient Monitoring

Health AllianceTM, the health plan of Illinois-based Carle Health, is leading the way for rising-risk populations by launching a remote patient monitoring program powered by CareSignal. This program is part of Hally® health, the member-facing telehealth app Health Alliance uses to give its members access to tools and resources for their well-being. The partnership between Health Alliance and CareSignal aims to reduce readmissions, reduce spending, and improve members’ experience while increasing health literacy and empowering members to better self-manage their chronic conditions.

The partnership is serving shared Medicare and Commercial populations and is expanding to Carle Health’s Accountable Care Organization.

CareSignal’s scalable remote patient monitoring technology sends evidence-based questions directly via phone call or text message to the member. Questions focus on the member’s health status and encourage adherence to their health management plan through reporting of symptoms and daily habits while monitoring changes in risk status. Indications of worsening symptoms can trigger real-time alerts to allow for intervention, including proactive health coaching and escalation of care. Health Alliance is currently prioritizing Chronic Obstructive Respiratory Disease congestive heart failure, hypertension, asthma, and post-discharge care, and is hoping to add additional populations.

Health Alliance is also bolstering its care coordination team with CareSignal’s virtual care navigators enabling them to manage a larger member population without needing additional staff. VCNs are the first to intervene with members before escalating to the Health Alliance care coordination team and/or primary care physician. By handling routine review, education and scheduling tasks when needed, the VCNs enable the Health Alliance care coordination team to work at the top of their license. The program provides increased support for members without increased touch points from providers.

Cardinal Health Announces Governance Enhancements and Shareholder Value Creation Initiatives

Cardinal Health announced initiatives aimed at positioning the Company for long-term success, building on Cardinal Health's previously announced growth plans. These initiatives have benefited from input received from Elliott Investment Management L.P. In connection with these initiatives, Elliott has also entered into a cooperation agreement with Cardinal Health, and Elliott's Steven Barg will join the Company's Board of Directors. As part of Cardinal Health's ongoing commitment to Board refreshment and the initiatives announced today, the Governance and Sustainability Committee of the Board has recommended and the Board has approved the appointment of four new independent directors: Steven Barg, Michelle Brennan, Sujatha Chandrasekaran and Christine Mundkur. With these immediate appointments, Cardinal Health's Board will be made up of 15 Directors, 14 of whom are independent under the Company's director independence standards. Two existing Directors – Dean Scarborough and John Weiland – have announced their intentions to conclude their Board service at the end of their previously elected terms this fall and will not stand for re-election at Cardinal Health's 2022 Annual Meeting, resulting in a 13-member Board. His science-based expertise and educational experience, alongside his international standing, will help educate investors and the general markets on the significance and industry importance of ATH-led initiatives,” said Centilli. Cronin serves as professor of strength and conditioning at the Sports Performance Research Institute of New Zealand (SPRINZ) at Auckland University of Technology (AUT). His research interests are in human movement, particularly around strengthening of muscle, injury resistance, return to play and high-performance sports. Cronin’s research has been published in more than 400 peer-reviewed papers. AUT and ATH have a long-standing history. Post-graduate fellows from AUT conduct research at ATH as part of a team involving coaches and select students, as well as collaborate on sport technology development. “I know the organization well, and my research interests and expertise align well with ATH’s interest in sporting performance and allied health, so I’m very excited to be advising ATH about the latest science-based studies and learnings to support their people, programs and products,” Cronin said.
