On January 26, 2023, Innovaccer, Inc., a data platform for healthcare, announced that it had entered a multiyear partnership with UpStream Healthcare, a value-based primary healthcare provider solution. This partnership entails UpStream more than doubling its population on Innovaccer's cloud-native platform in the coming year, scaling its exceptional value-based care model across the U.S.
In April 2022, Innovaccer announced that UpStream Healthcare had selected the Innovaccer Health Cloud, Application Suite and Data Activation Platform to speed up its mission to transform senior healthcare with a full-risk model that improves outcomes, health equity, and care costs. Medicare and Medicare Advantage members on the UpStream platform have a 20-30% lower total cost of care and a 4.5-star quality rating.
The company's platform physically embeds a pharmacist-led care team in every primary care physician's office and nurses who visit Medicare-eligible seniors at home. Adding to the point-of-care clinicians, other team members will close any clinical and quality gaps to care for all seniors in full-risk financial agreements.

Cardinal Health recently announced a partnership with Signify Health to provide clinical and medication management services in-home through its OutcomesTM business. This partnership will support the prescription-to-pharmacy-to-home treatment journey for more than 2.3 million members across the US by reducing costs and eliminating care gaps.
Numerous members do not follow their prescribed medications, according to studies. By combining Signify Health's mobile network of 11,000+ credentialed clinicians with Outcomes' industry-leading OutcomesOneTM clinical care platform and omnichannel network of payers and pharmacies, this collaboration will assist in solving medication management challenges for members of the health plan. It will achieve this by providing coordinated care from Signify Health clinicians, who will also perform evaluations of those members at-home.
Signify Health can use the OutcomesOneTM platform to determine and address patients' medication therapy management requirements based on the information gathered during in-home evaluations. The collaboration will initially focus on interventions advised for Medicare Advantage members of mutual clients and may expand to include additional services, like clinical interventions and population health programs.

On January 10, 2023, Vivante Health revealed the expansion of its digestive health platform, GIThrive, by including a cognitive behavioral therapy (CBT) digital therapeutic that aims to help users address conditions such as irritable bowel syndrome (IBS).
The new feature, known as IThrive, becomes the first CBT solution to be incorporated into a holistic digestive health platform and has been shown to reduce healthcare costs while improving participants' well-being.
Informed by the specific conditions of GIThrive's users, the new feature will let users benefit from a host of online tools for CBT, such as individual attention from a dedicated health coach and registered dietitian, daily audio sessions for ten weeks that teach CBT skills and techniques, guided meditation and breathing techniques for digestive health, evidence-based coping strategies, journaling, and other daily exercises.
Developed by clinical health psychologists, all IThrive components follow clinically validated CBT best practices to assist users in adjusting their behaviors and changing dysfunctional thinking patterns to help relieve symptoms of IBS and other digestive disorders.
Multiple independent studies have shown that CBT effectively relieves IBS symptoms, including one randomized controlled trial, in which, 90% of patients who used online CBT reported symptom improvement. In the latter study, 60% of participants reported a significant improvement in their symptoms.
IThrive is the first in a series of digital therapeutics that Vivante intends to add to its gut health platform to provide increasingly customized solutions for digestive disease patients.
